  • Staff Verification (PDF)
    For faculty and staff only. In the event one of four accepted forms of photo identification are not available when a new or replacement KUID (KU Card) is needed, or the name on your photo identification does not match the name in the payroll system, the Staff Verification form can be used as a means of verifying your identity so that a KU Card can be issued. Please complete the form and then visit the Payroll Office. They will verify your signature and photo, and fax the form to the Card Center
